Below is an outline of the steps involved in starting the Scholar Athlete application process

Page 1 – Scholar Athlete Form. ONLY ONE FORM PER PLAYER. Please fill out ALL required fields.
Page 2 – Your Cart Contains. Remove option available if there are more than ONE entry in your cart.
Page 3 – Check Out.
Page 4 – Credit Card Payment.
Page 5 – Order Confirmation. Player can click on the "print" option to print their order.

TIP: If you see more than one item in your shopping cart, please use the "remove" option to delete unwanted items

The Scholar Athlete Award is self funding, with the application fee covering costs associated with the management and fulfillment of the program. While the costs of the program are fixed, in order to ensure the Scholar Athlete Award remains accessible to all qualified students, regardless of individual family circumstances, we are pleased to offer the option to have the application fee waived if necessary.

The application deadline, including submission of your official school transcript to US Squash, is August 1, 2014.

Authorization:
I give my permission for US Squash to contact my High School for confirmation of my transcript. I also give authorization for my name, age, ranking and squash club or program to be included on any US Squash publication or literature, including the website.
